Product Submission Procedures

Samples submitted for testing cannot be hazardous. Biological samples must be submitted with a statement from your organization that the sample has been screened for any potentially hazardous biological components.

Follow these steps when submitting a product for testing:

1. Click on the 'Download Excel Spreadsheet' link below. Save the original file as a new Excel document where you can find it easily.

2. Please complete all the information in the blue colored boxes of the form.

3. After completing, please save this form to your computer using your purchase order number.xls as the name.

4. Email the filled out form as an attachment to service@mobio.com (Subject should read: Sample Submission)

5. Please submit samples with a SIGNED printed copy of completed form to the following address:
