Submit Samples
Product Submission Procedures
Samples submitted for testing cannot be hazardous. Biological samples must be submitted with a statement from your organization that the sample has been screened for any potentially hazardous biological components.Follow these steps when submitting a product for testing:
- 1. Click on the 'Download Excel Spreadsheet' link below. Save the original file as a new Excel document where you can find it easily.
- 2. Please complete all the information in the blue colored boxes of the form.
- 3. After completing, please save this form to your computer using your purchase order number.xls as the name.
- 4. Email the filled out form as an attachment to service@mobio.com (Subject should read: Sample Submission)
- 5. Please submit samples with a SIGNED printed copy of completed form to the following address: Mo Bio Laboratories, Inc.
